and Tributes Program

Honor Someone You Love, Remember Someone You Miss by Supporting the Vital Research Needed to Find a Cure for Diabetes and Its Complications.
The JDRF Memorials and Tributes Program offers friends of JDRF a thoughtful, caring way to remember a loved one or celebrate a special occasion while contributing to our vital mission to find a cure for diabetes.

There are so many reasons to give:

• Celebrate a birthday or other milestone
• Make a holiday really special
• Honor an achievement
• Send "get well" wishes
• Recognize a wedding anniversary
• Show a friend or family member who has lost a loved one how much you care

You can also use this program to raise money by asking others to give to JDRF in lieu of gifts for birthdays, graduations, weddings, anniversaries, or any other celebration in your own life.  Some families have raised thousands of dollars this way -- and it all supports diabetes research.

It's easy to participate!
We will need the following information:

• Your contact information
• The name of the person in whose honor or memory you are making the gift.
• The name and address of the person/people who should receive notification of your gift (for example, the honoree or surviving
family members)
• Your donation (credit card number or check)

To make a donation online, use the online memorial/tribute donation form

To make a donation by fax or mail, print and fill out the this form
and fax or mail your donation with the form to the address below.

You can also call the office at 312/ 670-0313 to make your donation (with a credit card) over the phone.

Once we process your information we will send you an acknowledgement letter. We will also send a card to the honoree or next of kin notifying them that you have made a memorial or tribute gift (we do not disclose the amount of the gift). Gifts are processed and acknowledged within two to four business days of receipt.

To order Wedding Favors, please read the following information:

We offer white 3 x 5 cards, folded horizontally, that feature our logo on the front. JDRF staff will print a special message to your guests on the inside.

Please limit your text to three lines.

Suggested text: “In lieu of favors, we have made a contribution to the Juvenile Diabetes Research Foundation in honor of our family and friends. Thank you for joining us in the celebration of our marriage.”
